  • I understand that Howl at the Moon Writers Jam may use my photograph, video, sound recording and my name, bio and intro in conjunction with these events and any related website(s) used to facilitate this program.  
  • I understand that Howl at the Moon Writers Jam may edit my original work(s); however, it is agreed that I will retain a copyright interest in such edited works. ​
I GIVE PERMISSION to be contacted via email, and for my photograph, video/audio recording, name, the introduction or biography I submit to be published, edited, and/or publicized as part of these events.
